Still some Senior food voucher’s available

For those who missed the opportunity earlier this month to get a Senior Nutrition Farmer’s Market Food Voucher, there is good news.

Pride Community Services has announced they still have approximately 150 vouchers left to distribute.

To be eligible for the coupons, you must be 60 plus years old and a resident of Logan County. To apply, bring identification, a tax receipt or driver’s license for proof of residence and age.

Home bound seniors may receive and complete a Proxy Authorization form by calling 304-752-6868 or by having someone visit their local senior center to pick up one. These must be filled out and signed by the participant. Hand written proxy authorization can be sent, but must contain the following information, that can be verified: participants name, birthdate, address, yearly income, number in household, phone number, county of residence and signature of applicant. It must also contain Proxy’s (person authorized to pick up and spend) name, address, phone number and signature.

Coupons are on a first come, first served basis. Incomplete proxy Authorizations will not be processed until completed. Only one set per household.

To sign up for the voucher, visit the Earl Jarvis Senior Enrichment Center, 699 Stratton Street, Logan, from 9 a.m.-noon and 1-3:30 p.m.
