PRIDE Community Services is conducting a survey to determine how to plan for meeting community needs. During the Tuesday, Feb. 5, basketball game between Logan High School and Lincoln County High School, at Logan, PRIDE will have a table set up to distribute Community Needs Survey forms.

Each person returning a completed survey will be given a ticket and entered to win a $50 Wal-Mart Gift Card. Surveys will be available from 5:30 p.m., when the junior varsity game begins, until half time of the varsity game.

The questions are easy to answer and the entire process only takes a few minutes. There are no right or wrong answers to the questions and no personal information is required. The questions are about education, employment, housing, health, nutrition, use of income, transportation, emergency situations and children’s needs.

All surveys must be completed and returned by half time of the varsity game to be eligible to win the $50 gift card. During half time, the winner of the gift card will be announced.

Logan County residents attending the game are encouraged to stop by our table and fill out a survey to help Logan County and get a chance to win a $50 gift card. Anyone 12 years old and older can fill out a survey and be entered to win the gift card.
